§ 11.0704. Reporting.
11.0704(1) (1) Contributions. 11.0704(1)(a) (a) Each conduit shall make full reports, upon a form prescribed by the commission and certified by the administrator as required under s. 11.0103 (3) (c) , providing the following information covering the period since the last date covered on the previous report: 11.0704(1)(a)1. 1.
