§ HS.248.022. APPLICATION
APPLICATION. (a) An applicant for a license must submit an application to the department on a form prescribed by the department and in accordance with department rules. (b) Each application must be accompanied by a nonrefundable license fee in an amount set by the executive commissioner by rule. (c) The department may require that an application be approved by the local health authority or other local official for compliance with municipal ordinances on building construction, fire prevention, and sanitation.
