§ GV.4004.103. APPLICATION FOR REGISTRATION
APPLICATION FOR REGISTRATION. The application described by Sections 4004.101 and 4004.102 must: (1) be in the form the commissioner prescribes; (2) state: (A) the residence and address of the person whose registration as an agent or investment adviser representative is requested through the application; and (B) any other information relating to that person's previous history, record, and associations that the commissioner may require; and (3) be signed and sworn to by the person whose registration as an agent or investment adviser representative is requested through the application.
