§ 4717.07.
(A) The board of embalmers and funeral directors shall charge and collect the following
fees: (1) For applying for an initial or biennial renewal of an embalmer's or funeral director's
license, or a reactivation of a license as described in division (H) of section 4717.05 of the Revised Code , two hundred dollars; (2) For applying for an embalmer or funeral director certificate of apprenticeship, thirty-five
dollars; (3) For the application to take the examination for a license to practice as an embalmer
or funeral director, or to retake a section of the examination, thirty-five dollars; (4) For applying for an initial license to operate a funeral home, four hundred dollars
and biennial renewal of a license to operate a funeral home, four hundred dollars; (5) For the reinstatement of a lapsed embalmer's or funeral director's license, the renewal
fee prescribed in division (A)(1) of this section plus fifty dollars for each month
or portion of a month the license is lapsed, but not more than one thousand dollars; (6) For the reinstatement of a lapsed license to operate a funeral home, the renewal
fee prescribed in division (A)(4) of this section plus fifty dollars for each month
or portion of a month the license is lapsed until reinstatement, but not more than
one thousand dollars; (7) For applying for a license to operate an embalming facility, four hundred dollars
and biennial renewal of a license to operate an embalming facility, four hundred dollars; (8) For the reinstatement of a lapsed license to operate an embalming facility, the renewal
fee prescribed in division (A)(7) of this section plus fifty dollars for each month
or portion of a month the license is lapsed until reinstatement, but not more than
one thousand dollars; (9) For applying for a license to operate a crematory facility, four hundred dollars
and biennial renewal of a license to operate a crematory facility, four hundred dollars; (10) For the reinstatement of a lapsed license to operate a crematory facility, the renewal
fee prescribed in division (A)(9) of this section plus fifty dollars for each month
or portion of a month the license is lapsed until reinstatement, but not more than
five hundred dollars; (11) For applying for the initial or biennial renewal of a crematory operator permit,
one hundred fifty dollars; (12) For the reinstatement of a lapsed crematory operator permit, the renewal fee prescribed
in division (A)(11) of this section plus fifty dollars for each month or portion of
a month the permit is lapsed, but not more than five hundred dollars; (13) For the issuance of a duplicate of a license issued under this chapter, ten dollars; (14) For each preneed funeral contract sold in the state other than those funded by the
assignment of an existing insurance policy, ten dollars. (B) In addition to the fees set forth in division (A) of this section, an applicant shall
pay the examination fee assessed by any examining agency the board uses for any section
of an examination required under this chapter. (C) Subject to the approval of the controlling board, the board of embalmers and funeral
directors may establish fees in excess of the amounts set forth in this section, provided
that these fees do not exceed the amounts set forth in this section by more than fifty
per cent.
