§ 3957.08.
(A) The superintendent of insurance shall provide a renewal notice to each licensee not
later than the first day of May each year. (B) A licensee may renew its pharmacy benefit manager license by applying to the superintendent,
in the form and manner prescribed by the superintendent, and paying a renewal fee
of three thousand dollars before the date the license expires. A licensee shall not apply for a license renewal more than ninety days before the
date the license expires. (C) In the event that a licensee fails to apply for renewal and pay the renewal fee before
the date the license expires, the license shall expire on the expiration date, and
the former licensee is not authorized to operate as a pharmacy benefit manager in
this state beginning on that date. A person whose license is expired may apply to reinstate the license in the same
manner as a license renewal under division (B) of this section, except that the filing
fee is one and one-half times the renewal fee under division (B) of this section.
