§ 45:6-18.
No dentist and no professional service corporation engaged in the practice of dentistry
in this State shall charge a patient an extra fee for services rendered in completing
a dental claim form in connection with a health insurance policy. Any person violating this act shall be subject to a fine of $100.00 for each offense. Such penalty shall be collected and enforced by summary proceedings pursuant to “the
penalty enforcement law ( N.J.S. 2A:58-1 et seq. ). The Superior Court and municipal court shall have jurisdiction within its territory
of such proceedings. Process shall be either in the nature of a summons or warrant and shall issue in
the name of the State, upon the complaint of the New Jersey State Board of Dentistry,
as plaintiff.
