§ 65.
Endorsement. (a) The Department may, upon application in writing on forms or electronically accompanied by the required fee, issue a license as a perfusionist to an applicant who is a perfusionist licensed under the law of another state, the District of Columbia, territory, or country, if the requirements for licensure in that jurisdiction were, at the date of original licensure, substantially equivalent to the requirements in force in this State. (b) An applicant who holds a current certificate as a certified clinical perfusionist issued by the American Board of Cardiovascular Perfusion, or its equivalent, as approved by the Department, prior to January 1, 1999 may apply for endorsement as stated in subsection (a) of this Section. (c) If the accuracy of any submitted documentation or relevance or sufficiency of the course work or experience is questioned by the Department or the Board because of a lack of information, discrepancies, or conflicts in information given, or a need for clarification, the applicant seeking licensure may be required to provide additional information. (d) Applicants have 3 years from the date of application to complete the application process. If the process has not been completed in 3 years, the application shall be denied, the fee forfeited, and the applicant must reapply and meet the requirements in effect at the time of reapplication.
